Brand Label Registration

The following information is required with the State of Missouri.
Missouri is a three-tier State.

1. Solicitor - Must be licensed with the Division of Alcohol & Tobacco Control using the Out State Solicitor Application (contact: Diane Markway 573.751.5444 email: Diane.Markway@dps.mo.gov for licensing paperwork). When approved this license will allow the Solicitor to register products to be shipped into Missouri to a licensed Wholesaler. Monthly tax reporting forms are submitted by the Solicitor (contact Charlene Mitchem 573.751.5452 email: Charlene.Mitchem@dps.mo.gov for questions on tax reporting).
To register products the following must be submitted: (contact Debbie Nichols 573.751.4073 email: Debbie.Nichols@dps.mo.gov for Label Registration questions)
a. Application for Primary American Source Registration Form (in reference to section 311.275 RSMo, there can only be one Primary Source of Supply) Electronic Forms can be accessed at the following address: http://www.atc.dps.mo.gov/forms.html
b. Original Labels taped or glued to a 8 1/2 x 11 sheet of paper

Corresponding approved TTB-COLA
Malt Products: Original labeled sample of product, if the alcohol content is not approved on the TTB-COLA
Wholesaler Appointment Letter, if registering Malt product - list territorial agreement.

2. Wholesaler
Must be licensed with the Division of Alcohol & Tobacco Control using the Wholesaler Application. When approved this license will allow the Wholesaler to post pricing and sell to licensed Retailer. Wholesale licensees can be accessed at http://www.atc.dps.mo.gov/Reports.htm

3. Retailer
Must be licensed with the Division of Alcohol & Tobacco Control using the Retailer Application. When approved this license will allow the Retailer to sell product.

When submitting paperwork for a label revision please include the following:

* Cover Letter stating revision, list brand name, brand number if available and changes to the label
* 1 set of original labels glued to a blank sheet of paper
* Copy of new TTB Federal Label Approval
