Electronic Forms

NOTE: Electronic Forms will not work on a Macintosh Computer. Additionally, they may not work correctly on Windows Vista. If you have either of these operating systems, you may need to contact the office to have the forms sent to you. 573-751-2333.

If you are a first time user of electronic forms, please read the directions below. You may also follow the instructions located at the following address: http://www.oa.mo.gov/gs/form/fm_guide.htm.
It is necessary to install the "Setup" (click Setup to install) file prior to downloading any of the individual forms below. After installing the Setup file, you will need to re-boot your computer before installing any individual forms. It is not necessary to re-boot only for individual forms.
